For your convenience, we will bill your PIP insurance company for your medical massage treatment with a prescription from your physician.

What do I need to schedule an appointment to start my medical massage treatment?

Please call the office with the following information: (we need to verify an open claim for you)

  • Date of accident
  • Insurance company name, mailing address & phone number
  • Adjuster’s name
  • Claim #

We will immediately verify the insurance and call you to schedule your first appointment.  This is usually within 24 hrs.  You will need to bring  prescription from your physician or chiropractor on your first visit, with the following information on it:

  • Date
  • Frequency of Treatment (ex: 1-2 sessions/wk for 4 wks.)
  • Diagnosis (reason for treatment)
